CMSCreate an Organisation

Create an Organisation

Any signed-in user can create an organisation at any time, whether it is their first or their fifth. The creator becomes its first administrator automatically.

Reach the form from /organisations/new, from Create organisation on the organisations screen, on the welcome screen or in the sidebar’s organisation switcher.

No Medplum server? Use the link under the form, “Don’t have a Medplum server? Request a Medbot-hosted one”, and Medbot will set one up for you. See Medbot-Hosted Organisations.

Before you start

You need an OAuth client on the Medplum server this clinic will use:

  1. Sign up at Medplum  if the clinic has no project yet.
  2. Create an OAuth client in Medplum.
  3. Copy its Client ID and Client Secret.

These go into the form below, not into an environment variable. The CMS holds no Medplum credentials of its own.

The wizard

The form is a short wizard, headed “Step N of M” with the step’s name. The number of steps depends on the country.

Step 1: Organisation

FieldRequiredNotes
Organisation nameYesThe clinic’s name, e.g. “Orchard Clinic”.
CountryYesGuessed from your browser, falling back to Singapore. Decides which country-specific integrations are offered.
Time zoneNoGuessed from your browser.

Next stays disabled until the name and country are set.

Step 2: Singapore integrations (Singapore only)

Shown only when the country is Singapore. Choose the SmartCMS modules the clinic intends to use, or click Use GP starter for the usual general-practice set (CHAS, Medisave, National Immunisation Registry, Disease notification). Skip for now leaves them empty; they can be chosen later in settings.

Choosing a module does not connect anything by itself: each clinic location still needs its HE code and Synapxe’s approval.

Step 3: Medplum server

FieldRequiredNotes
Medplum base URLNoDefaults to https://api.medplum.com. Set it for a self-hosted Medplum.
Medplum client IDYesFrom step 2 of “Before you start”.
Medplum client secretYesWrite-only: it is never shown back to you.

Click Create organisation.

What happens on submit

The credentials are validated against Medplum before anything is created, so a bad client id or secret fails the form rather than leaving a half-built organisation behind. Once they check out, the backend creates, in one transaction:

  • the organisation itself, with a generated slug and join code, its country, time zone and any SmartCMS modules
  • the creator’s membership as admin, with their name and email
  • the encrypted Medplum credentials
  • a default clinic location, “Main Clinic”

It then creates the admin’s own Practitioner record on that organisation’s Medplum and stores its id. If that step fails, the organisation is deactivated and the error surfaced rather than leaving an admin who cannot act clinically.

The API refuses SmartCMS modules for any country other than Singapore.

After creating

The new organisation becomes the current one and you land on Add your team and patients (/organisations/<id>/seed?setup=1), where you can bring in staff and patients from a CSV file. See Seeding Staff & Patients. Skip for now takes you to the CMS home screen instead.

If you chose any Singapore integrations, the same page shows a Connect SmartCMS card with a Review integrations link: each location needs its HE code and Synapxe’s approval before it connects. See Singapore Registration and SmartCMS Settings.

From there:

There is no manual role edit anywhere in the app and no separate “make me an admin” step. The first admin is whoever created the organisation; every admin after that is one an existing admin added.
